Network Extension Request Form

This form should be used for anyone wishing to add a Wireless Access Point, Gaming Console, Hub, or Switch to the campus network. 

Please note that a personal wireless access point is only permitted in residence halls if the hall is not covered by Brown's wireless network, it meets the listed guidelines, and you have been granted approval from a CIS staff person prior to doing the installation. Please see  http://cis.brown.edu/wireless for a current list of affected residence halls.

This form is used to inform the appropriate CIS staff about your plans, and someone will get back to you regarding the request. For requests related to the wiring of taps in a building, please fill out the Wiring Change Request form.

1. Tap where device is to be connected to the campus network. Please specify Building, Room, and TapID (on the label on the wall plate).

2. Who will be the owner of the device?

3. Please provide a Brown phone number for the device owner.
4. Please provide an email address at Brown for the device owner.
5. What is the MAC address of the device?
6. What type of device would you like to install?
7. (For Wireless Access Points only) What is the SSID you plan to use? (You cannot use Brown).
8. What IP address range will you serve? (You cannot use Brown address space.)
9. Is there anything else you think we should know about your planned installation?
