My research has been built on two platforms, clinical trials in stroke prevention, and translational biomarker research. I have had continuous NIH and foundation support for this research throughout my career. I was previously the principal investigator of the NINDS P50 Partners Specialized Program of Translational Research in Acute Stroke (SPOTRIAS) and the American Heart Association supported Harvard Bugher Center for Stroke Prevention and Genetics Research, and I am currently the Principal Neurologist on the Insulin Resistance Intervention after Stroke trial. I am co-PI of the ARISE trial within the Partners SPOTRIAS.


I received my undergraduate degree in English and American Literature from Brown University in 1987, MD from Brown University School of Medicine (1990), and a Masters in Public Health (1996) from the Harvard University School of Public Health. I completed my medical internship at the Miriam Hospital (1991), and a Neurology Residency (1994) and Fellowship in Cerebrovascular Disease at Rhode Island Hospital (1995). I joined the faculty of the MGH Stroke Service in 1995 and served as Director of the MGH Stroke Service from 2007-2012. In August 2012, I was named Chair of Neurology at the Alpert Medical School of Brown University.

Clinical and translational stroke research has been a major focus of my career, and I have enjoyed success in developing collaborative multispecialty initiatives.

Research Description

My research endeavors have defined optimal antithrombotic therapy for stroke prevention, clarified the role of high dose vitamins for treatment of hyperhomocysteinemia, and provided insight into the pathophysiology of ischemic stroke. I was the principal investigator of the NINDS P50 Partners Specialized Program of Translational Research in Acute Stroke (SPOTRIAS) and the American Heart Association supported Harvard Bugher Center for Stroke Prevention and Genetics Research, and am currently the Principal Neurologist on the Insulin Resistance Intervention after Stroke trial. I am committed to mentoring patient-oriented research of medical students, residents, and fellows.

My current research focus has been on the interaction between oxidative stress and matrix metalloproteinases to better define the mechanism of injury in acute stroke, and identify potential targets for intervention. We are examining dietary factors as they contribute to basal antioxidant capacity, and exploring in vivo effects of acute and chronic inflammation on oxidative stress. Furthermore, we have developed a unique semi-automated process for analyzing white matter disease, a silent and insidious manifestation of cerebrovascular disease, which will enable us to identify the contributions of genetic polymorphisms, oxidative stress, and cerebral blood flow on the pathophysiology of this process.

Insulin Resistance Intervention after Stroke Trial (IRIS)
The goal of the study is to study the effects of pioglitazone on insulin resistance for preventing recurrent strokes. I provide neurological leadership for the study and enroll patients across the Partners network.

Development of Annexin A2 plus tPA as a novel stroke thrombolytic therapy
The major goals of this project are to test 8 specific aims going from translational experiments in well- controlled rat embolic stroke models for therapeutic efficacy, pharmacokinetic and toxicity profiles, to an IND application for the optimal combination dose of rA2 plus tPA at the end of our 5-year project. I will advise on translational components of the study, participate in the IND application, and help develop the protocol for human use.

Chagas disease (CD) is a significant public health concern, affecting over 18 million individuals and major cause of heart disease, stroke and cognitive impairment in Latin America. Chronic inflammation from CD both increases stroke risk and is associated with brain atrophy. Thus, the main objectives of the current project are to investigate brain involvement in CD through brain imaging and systemic biomarkers; and to evaluate the efficacy of antiplatelet treatment in decreasing microembolization rate to the brain. I developed the protocol, will oversee study progress, and will participate in data analysis.

Teaching Experience

Teaching and training a cadre of neurologists and clinical researchers has been a major focus of my efforts.I lecture in the first year Neuroscience course and teach neurological assessment skills. I routinely speak on stroke epidemiology and prevention.
