Athletic Camps on the Brown Bears Campus
Forms Required for all Sports Camps Students
All campers are required to fill out the acceptance and medical authorization form.
NOTE: Only overnight campers need to fill out the immunization form.
- Acceptance & Code of Conduct Form
Form is to be completed by the student if over 18 years of age and the parent or guardian if the camper is under 18 years of age. Parent/guardian must sign pages 1 and 3, camper must sign page 1.
- Immunization History Form
Please fill out the top. The remainder of this form is to be completed and signed by your medical provider.
- Medical Authorization Form
Complete both sides of this form. The camper and parent must sign the back side of the form, and submit with a copy of both sides of your health insurance card. Campers are not required to have a physical exam. Information regarding purchasing a limited University Health Insurance Policy for the duration of your camp will be available soon.
- Disability and Medical Accommodations Form (if applicable)
This form should be filled out if a camper needs medical or disability accommodations, including significant food allergies that require special meals (for camps that are providing meals in the dining hall). Please be aware that all accommodation requests will be considered; however, there may be limits as to what can be provided without sufficient notice. Student and Employee Accommodations Services (SEAS) recommend a minimum of two weeks notice. The student and parent must sign the bottom of this form and submit directly to the SEASOffice (see form for fax and address). - Guest Allergy Form (if applicable)
Campers should only fill this form out if there are significant food allergies, and the camper has also filled out the Disability and Medical Accommodations Form above.
If you are attending Summer@Brown and have already submitted Immunization History and Medical Authorization Forms, you do not need to re-submit. Please submit only the Acceptance & Code of Conduct Form.
