Athletic Camps on the Brown Bears Campus
Forms Required for all Sports Campers at check-in
All Campers are required to submit the Sports Camps Acceptance Form , Medical Authorization Form and if an overnight camper the Immunization History Form. All forms will be collected at camp check in. Without these forms completed in their entirety, campers will not be permitted to participate in any camp activity.
- Sports Camps Acceptance Form
Form is to be completed by the student if over 18 years of age and the parent or guardian if the camper is under 18 years of age. Parent/guardian must sign pages 1 and 3, camper must sign page 1.
- Immunization History Form
NOTE: Only overnight campers need to fill out the immunization form. Please fill out the top portion of the form. The remainder of this form is to be completed and signed by your medical provider.
- Medical Authorization Form
Complete both sides of this form. Campers are not required to have a physical exam. The camper and parent must sign the back side of the form, and submit with a copy of both sides of your health insurance card. If you cannot provide proof of insurance with a US carrier, you must purchase a limited health insurance policy for $15.
NOTE: If your camper needs medical or disability accommodations, including significant food allergies (such as gluten, wheat, milk soy, or eggs) that require special meals, please fill out the Disability and Medical Accommodations Form and/or Guest Allergy Form below. Please be aware that all accommodation requests will be considered; however, there may be limits as to what can be provided without sufficient notice. Please note forms are only accessed in the case of an emergency.
Forms Required for all Sports Campers at least 2 weeks prior to check-in if applicable
These forms should be returned directly to SEAS office at least two weeks prior to the start of camp see form for fax and address).
- Disability and Medical Accommodations Form (if applicable)
This form should be filled out if a camper needs medical or disability accommodations, including significant food allergies (such as gluten, wheat, milk soy, or eggs) that require special meals (for camps that are providing meals in the dining hall). Please be aware that all accommodation requests will be considered; however, there may be limits as to what can be provided without sufficient notice. Student and Employee Accommodations Services (SEAS) recommend a minimum of two weeks notice. The student and parent must sign the bottom of this form and submit directly to the SEAS Office (see form for fax and address).
- Guest Allergy Form (if applicable)
Campers should only fill this form out if there are significant food allergies (such as gluten, wheat, milk soy, or eggs), and the camper has also filled out the Disability and Medical Accommodations Form above.
If you are attending Summer@Brown and have already submitted Immunization History and Medical Authorization Forms, you do not need to re-submit. Please submit only the Sports Camps Acceptance Form at your camp check in.

When contacting, please be sure to include: the camper’s name, the name of the camp and the dates of attendance.
