Acute demyelinating disease

 

 

Staining for neurofilament protein reveals relative axonal preservation within the lesion, consistent with demyelinating

disease.  If there were widespread axonal destruction, the lesion would have been more consistent with an infarct. 

Because of the acute presentation, it is impossible to say whether this represents a first attack of multiple sclerosis

(MS) or a single attack of acute disseminated encephalomyelitis (ADEM).  Only time will answer that question.
